                                         Proof of address : 

• Applicant's ration card
• Bank account pass book
  
• Parent's passport copy (if holding) in case of minors 
• Student card – by Recognized Educational Institutions
• Photo Identify card – by Post office
  

                                          Proof of Date of Birth (attach one of the following): 

• Birth certificate issued by a Municipal Authority
• 10^th, +2 Mark sheet
• Affidavit sworn Notary stating date/place of birth
  

A Declaration affirming the particulars furnished in the application about the minor child as per ANNEXURE "H" (signed by both parents).

ANNEXURE"I"   - (Standard Affidavit) when a minor between 15-18 years of age applies for a full validity 10 year passport OR in case either parents who do not hold valid Indian passport while applying passport for their minor child.
